To ensure the accuracy and safe delivery of the new certificate, diplomates are required to provide the ABD with the manner in which their name should be printed on their certificate and the address to which the certificate should be delivered.
Please enter your name EXACTLY as you wish it to appear on your certificate, using the following format:
First name* Middle name/initial Last name*, Degree(s)
* indicates a required value
Example:
John Q. Smith, MD, PhD
Include post-nominal letters or professional designations (e.g. MD, MBA, PhD) as they are to appear on the certificate. Do not use punctuation and separate each degree with a comma.
